Clearsound Productions Ltd now requires a negative rapid lateral flow test result to be produced within 24hours before working/entering our studios, offices, warehouse or working onsite.

You should take a rapid lateral flow test twice a week (every 3 or 4 days) and complete a new Health Declaration Form for each test.

If you have a rapid lateral flow home test kit please fill out the information below.

If you don't have a rapid lateral flow home test kit you can order packs free of charge online HERE or please contact us. We will either send you a test in advance or ask you to do a test in your car outside our premises or onsite.

We will not allow anybody to start work unless they produce a negative test result and have completed this form.

After completing our health declaration you will be then asked to then submit your results to the NHS website. If working onsite proof of a negative lateral flow or PCR test maybe required by the end client or venue so please fill this out too and be prepared to show your results via the NHS COVID Pass on the NHS app OR a text message or email via NHS Track and Trace, shown on a device or a print-out.

I agree I will not come to site if I am suffering from any signs or symptoms or COVID- 19 such as high temperature, a new or continuous cough, a loss or change to my sense of smell or taste

I agree I will not come to site if I have been in close contact with or cared for someone diagnosed with Covid-19 within 10 days of coming to site, unless I am double vaccinated

I agree I will not come to site if I have been contacted by NHS Test & Trace within 10 days of coming to site and instructed to self-isolate

I understand I may need to show proof of a negative test result before entering site.

If you produce a positive test result or can't check all of the above, you must inform us immediately.

If you or anyone in your household tests positive, everyone in the household must:

self-isolate immediately
get a PCR test to confirm the result (a PCR test is a different type of coronavirus test)
